Wish List: How to write a reference letter

Continuing a vague urge, I wish someone would author a short article on how to write a reference letter, North American style, in particular for graduate admissions. This will help faculty members elsewhere avoid overly coy letters or ones that praise a student for pleasant character or yet others that merely list dry stats of classroom performance.
